Manufacturer Disclosure Statement for Medical Device Security (MDS2) forms are information-rich tools that allow you to understand, as the equipment owner, pertinent aspects of that device, including:

  • The types of EPHI stored on that device
  • Its intended environment
  • What, if any, external software has been validated by the manufacturer so as avoid the corruption of its data output, patient harm or complete shut down

The most recent MDS2 form was developed in 2008 for medical device manufacturers by HIMSS and NEMA.
